1. Collection and use of personal information

The Company will handle personal information after specifying the purpose of utilization thereof and will, when collecting the personal information, (1) notify the relevant person of the purpose of utilization thereof or publicly disclose it, (2) obtain the consent thereto, and (3) take other appropriate measures in conformity with laws, regulations, and other norms. Additionally, the Company will use the personal information within the scope of the purpose of utilization thereof.

2. Management and protection of personal information

The Company will take appropriate safety management measures and corrective measures to prevent unauthorized access to, loss, destruction, falsification, divulgence, etc., of the personal information in its possession. Moreover, the Company will not provide the data to any third party, except where the consent of the concerned person is obtained, or such provision is permitted by laws, regulations, or other norms.

3. Compliance with laws, regulations, and norms

The Company will comply with the Act on the Protection of Personal Information and other laws, regulations, and norms applicable to the personal information in its possession.

4. Management system and mechanism for the protection of personal information

The Company will establish, maintain, and continuously improve the management system and mechanism for the protection of personal information.

2. Purpose of utilization of Personal Information

The Company handles personal information in the course of the implementation of its business for the following purposes of utilization, or within the scope of other purposes that will be separately informed.

(1) Personal information relating to doctors, dentists, pharmacists, nurses, persons involved in medical institutions, persons involved in pharmacies and drugstores, other healthcare professionals, and researchers involved in medicine and pharmaceutical science

  • For the purpose of providing and collecting information about the proper use of products that the Company imports, manufactures, and sells (hereinafter referred to as the "Company Products")
  • For the purpose of providing and collecting information about the quality, safety, or efficacy of Company Products

  • For the purpose of conducting investigations and research in the fields of medicine and pharmaceuticals
  • For the purpose of carrying out and supporting academic activities, such as research conferences and lecture meetings, in the fields of medicine and pharmaceuticals
  • For the purpose of authenticating the subscribers to the Internet site operated by the Company for medical personnel, and providing guidance, notices, and information to them
  • For the purpose of making applications, notifications, reports, etc., to the government and public offices, participating groups, etc.

(2) Personal information concerning users of contact points of the Company such as the Public Relations Department

  • For the purpose of examining and investigating the contents of inquiries, consultations, communications, etc., and dealing therewith, responding thereto, and recording such matters
  • For the purpose of communicating with and providing information to manufacturers or distributors of Company Products, as well as medical personnel, etc.
  • For the purpose of making notifications, reports, etc., to the government and public offices, participating groups, etc.

(3) Personal information concerning users of the Company's Internet site, applicants to the campaigns conducted by the Company, and the respondents to questionnaire investigations conducted by the Company

  • For the purpose of making communications, applications, reports, etc., of various kinds for carrying out the activities of the relevant organizations

(8) Personal information concerning journalists of newspapers, magazines, and other media

  • For the purpose of making communications of various kinds to carry out public relations activities, such as sending data and materials, including news releases

(9) Personal information concerning attorneys, accountants, tax accountants, judicial scriveners, etc.

  • For the purpose of making communications, requests, and consultations of various kinds to carry out the business purposes of the Company

3. Sharing of Personal Data

(1) Personal data contained in the Medical Database (hereinafter referred to as "MDB") managed and operated by Nihon Ultmarc Inc.

The Company shares the personal data contained in the MDB, which is managed and operated by Nihon Ultmarc Inc., with other companies (member companies) using the MDB of Nihon Ultmarc Inc. and other MDBs, for the purposes of utilization described in 2 (1). (2) Documents, etc., to be attached to requests for disclosure, etc.

Please download "A. Request form prescribed by the Company" according to the type of request, complete it by filling in all prescribed items, and send it with "B. Identification document." (3) Request for disclosure, etc., by a representative

In the case where the person who makes a request for disclosure, etc., is the statutory representative of a minor or an adult ward or the representative to whom the requesting person entrusts the request for disclosure, etc., please enclose the following documents (referred to in A or B) in the envelope, in addition to the documents described in (2) above.

B. In the case where the person making a request for disclosure, etc., is the representative to whom the requesting person entrusts the request for disclosure, etc.: all the documents below

  • A certificate of the seal impression of the requesting person
  • An original copy of the resident register of the representative and a copy of either the driver's license or passport of the representative, as documents for identification of the representative (If the representative has neither a driver's license nor a passport, a copy of either his/her health insurance card or pension book will suffice.)

(4) Charge for requests for disclosure, etc., and collection method

A person who makes (1) a request for notice of the purpose of utilization or (2) a request for disclosure with respect to retained personal data, needs to enclose a ¥800 postage stamp with the request documents in an envelope as a charge therefor, on a request-by-request basis. In this regard, two (2) requests made at the same time are regarded as one (1) request.

(5) Method of replying to requests for disclosure, etc.

A written reply will be mailed (by registered mail with return receipt requested).

(6) Cases where the Company declines a request for disclosure, etc.

Please understand in advance that the Company will decline a request for disclosure, etc., in the case where:

  • the requesting person or representative is not identifiable;
  • no power of representation can be confirmed with respect to a request made by a representative;
  • there is a flaw in the prescribed request documents;
  • the charge is not paid within the prescribed period;
  • the requested information item does not fall under retained personal data;
  • it is unnecessary to accommodate the request for disclosure, etc., pursuant to the provisions of the "Act on the Protection of Personal Information;" or
  • it violates any other law or regulation.

5. Inquiry / Consultation about Personal Information

6. Matters that Require Attention

  • The meanings of "personal information," "personal data," and "retained personal data" shall be the same as those defined in the "Act on the Protection of Personal Information."
  • The Company will review and endeavor to improve the "Policy for protection of personal information" in Section 1 and operations of individual items in Section 2 and subsequent sections, from time to time. Therefore, please understand in advance that the policy and descriptions in individual items will be changed without notice.
